HCPLAN Releases New Guidebook to Help Organizations Advance Patient Empowerment
New resource offers practical strategies to strengthen affordability, access, and navigation to support value-based care.
July 30, 2026
Today the Health Care Payment Learning & Action Network (HCPLAN) announced the release of its Patient Empowerment Guidebook: Pathways to Advancing Patient Empowerment — a practical resource designed to help organizations empower patients to take a more active role in their health.
Developed for providers, health plans, employers, and community-based organizations, the Guidebook focuses on three interconnected priorities: making care more affordable, ensuring patients can access the services they need, and helping them navigate the health care system with confidence.
The Guidebook provides actionable strategies organizations can use to advance patient empowerment, including redesigning financial incentives, increasing access to wellness activities, and delivering personalized reminders and nudges. It also highlights foundational actions, stakeholder roles, real-world examples, and implementation guidance to help organization move from awareness to action.
Rather than offering a one-size-fits-all approach, the Guidebook recognizes that patient empowerment looks different for every individual. It encourages organizations to consider factors such as health literacy, language, digital access, readiness, personal preferences, and trusted support networks when designing patient-centered strategies.
Gita Deo, Chief of Staff and Senior Advisor for the CMS Innovation Center, stated in reference to the Guidebook that, “From a CMMI perspective, what stands out is the focus on practical, actionable strategies — particularly across affordability, access, and navigation — that reflect what we know patients need to engage more meaningfully in their care.”
Organizations interested in strengthening patient empowerment and advancing value-based care are encouraged to explore the Guidebook and consider how its practical strategies can be applied within their programs and communities.
