The Health Care Payment Learning and Action Network’s (LAN) Health Equity Advisory Team (HEAT) has released a new guidance document, “Guidance for Health Care Entities Partnering with Community- Based Organizations: Addressing Health-Related Social Needs in Alternative Payment Models.”

Background: Previously, the HEAT identified partnerships between health entities and community-based organizations (CBOs) as an important driver of more equitable health outcomes in the design of alternative payment models (APMs) in their Theory of Change publication.
Now, the HEAT has developed this new guidance as a needed resource for health entities involved in designing and implementing APMs, organizing the content around four thematic categories derived from interviews with CBOs and community care hubs:
1. Collaborative Governance Grounded in Mutual Respect and Shared Decision-Making
2. Hubs and Neutral Conveners as Potential Conduits for Partnership
3. Building CBO Capacity and Infrastructure through Multi-Sector Partnership
4. Funding and Financing to Support Cross-Sectoral Collaboration to Address Social Needs
