Background: This guidance aims to scale promising practices for incentivizing health equity through APMs across the health care industry, with the goal of reducing inequities in care and outcomes. Drawing from real-world observations and promising multi-sector practices, this document outlines practical steps to operationalize accountable care initiatives that integrate incentives for health equity.
The document contains a set of promising practices for providers, health plans, and purchasers. These practices focus on five building blocks that are essential to supporting health equity through APMs and broader accountable care arrangements:
