The LAN’s Bold New Goals to Transform Health Care Payment
At the seventh annual LAN Summit on October 24, 2019, the LAN announced bold new goals to accelerate the shift to value-based care. The LAN has intentionally set these new goals to align with the current trends and state of payment reform, as well as goals and objectives for CMMI and CMS. These goals, while ambitious, are achievable with commitment and action from stakeholders across the care landscape.
New Goals Statement
The LAN aims to accelerate the percentage of US health care payments tied to quality and value in each market segment (Medicaid, Commercial, Medicare Advantage, Traditional Medicare) through the adoption of two-sided risk APMs—Categories 3B, 4A, 4B, and 4C from the LAN’s APM Framework as shown here. |

- Tie 15% of Medicaid payments to two-sided risk APMs by 2020, 25% by 2022, and 50% by 2025.
- Tie 15% of Commercial payments to two-sided risk APMs by 2020, 25% by 2022, and 50% by 2025.
- Tie 30% of Medicare Advantage payments to two-sided risk APMs by 2020, 50% by 2022, and 100% by 2025.
- Tie 30% of Traditional Medicare payments to two-sided risk APMs by 2020, 50% by 2022, and 100% by 2025.
A New Mission and Vision for the LAN
To complement the release of these new goals, the LAN also unveiled a new vision and mission at the LAN Summit. The LAN’s vision is for an American health care system that pays for value to the benefit of our patients and communities. To achieve this vision, the LAN’s mission is to accelerate the shift to value-based care in order to achieve better outcomes at lower cost.
The LAN has designed a two-page goal summary that breaks down our new goals, mission and vision for payment reform, and provides details on where the LAN will target its efforts to improve patient outcomes and reduce costs.
We have also produced a Frequently Asked Questions (FAQs) page and accompanying document to help you understand how the new goals differ from the LAN’s previous goals, how two-sided risk models will be measured, key definitions, and more. Catch Up on the 2019 LAN Summit!
Hosted at the Hyatt Regency Washington on Capitol Hill on October 24, the seventh annual LAN Summit brought together more than 600 payers, providers, purchasers, policymakers, patients, and other stakeholders. Over the course of a tightly organized day, attendees shared resources, best practices, and mobilized around the theme of Aligning for Shared Accountability and the movement to two-sided risk models in accelerating the shift to value-based care.
The LAN Welcomes a New Co-Chair
At the LAN Summit, we announced an exciting change to our leadership, welcoming Dr. Marc Harrison, president and CEO of Intermountain Healthcare, as the new LAN CEO Forum co-chair. Dr. Harrison will join Dr. Mark McClellan, Director of the Duke-Margolis Center for Health Policy, as co-chair going forward, and the LAN is thrilled to have such an accomplished leader on board to drive change as we transition to a new focus and direction for the future. Dr. Mark Smith, who co-chaired the LAN Guiding Committee since its inception in 2015, will transition from the LAN at the end of this month, and we sincerely thank him for his service and invaluable role in steering our body of work to date.

A Steady Trend:
Catch Up on the 2019 APM Measurement Effort Results
The 2019 LAN Summit also featured the release of the 2019 APM Measurement Effort results. Every year, the LAN conducts a national effort to assess the adoption of APMs over time and track progress towards the LAN’s goals.
Since its start in 2015, the Measurement Effort has evolved to incorporate data from a large sample of payers that represent nearly 80% of covered Americans, and now serves as the most comprehensive snapshot available for measuring progress on payment reform. The first Measurement Effort reported that, in 2015, 23% of all health care payments were tied to APMs. That number increased to 29% in 2016, and to 34% in 2017.
As noted in the LAN’s press release following the 2019 LAN Summit, we’re pleased to announce that the results of our annual Measurement Effort showed that 35.8% of payments flowed through APMs—shared savings, shared risk, bundled payment, and population-based payments.

A Brief Analysis of the Results
Four years ago, one in four heath care payments flowed through an APM. Today, one in every three health care payments flows through an APM. Since the LAN started measuring the flow of health care payments, the percentage aligned to fee-for-service models, with no link to quality and value, has dropped from 62% in 2015 to 40% in 2018—a roughly 33% reduction in fee-for-service (FFS) payments.
Over the past four years, Category 2 FFS payments with links to quality and value have increased by roughly 60%, while Categories 3 and 4 payments have increased by roughly 56% over that time. This year’s Measurement Effort surveyed payers on the future of APM adoption, and 91% believe that APM activity will increase, 7% believe it will remain constant, and none believe that APM activity will decrease.

The LAN is thrilled to announce the official launch of two new Executive Forums: the CEO Forum and Care Transformation Forum (CTF). These Forums convene health care leaders committed to shaping the strategic direction for value-based payment in the US.Led by co-chairs Mark McClellan, Director of the Duke Margolis Center for Health Policy, and Marc Harrison, President and CEO of Intermountain Healthcare, the CEO Forum provides guidance on opportunities for strengthening incentives and capacity to accelerate the transition to two-sided risk models across markets. |

The complementary CTF is led by co-chairs Sachin Jain, President and CEO of CareMore Health and Aspire Healthcare, and William Shrank, Chief Medical Officer of Humana, and is designed to influence and shape care delivery transformation, focusing on identifying tools and strategies that prepare organizations for success in delivering high-quality care that improves patient outcomes and reduces costs across a multi-payer environment.
The CTF had its inaugural meeting earlier this month, and used its agenda to discuss social determinants of health; reducing ineffective care; increasing data transparency and interoperability; ensuring timely data and analytics capabilities; facilitating market shifts to value; and promoting population-specific approaches. Stay tuned to the LAN’s social channels and email updates for outcomes from that meeting and plans for future action.
