Coming Soon: Draft White Paper on Elective Joint Replacement | ||||||
The public comment period for the first draft white paper on episode payment for elective joint replacement opens on Friday, February 26 and closes on Monday, March 28. The Clinical Episode Payment (CEP) Work Group invites your input over the next month to help ensure the paper reflects the latest and most comprehensive knowledge from the health care payment field.
The draft white paper, entitled Accelerating and Aligning Clinical Episode Payment Models: Elective Joint Replacement, provides recommendations for how to design patient-centered episode payment for elective hip and knee replacement. More specifically, the paper discusses the Work Group’s recommendations for those design elements and operational considerations that support clinical episode payment adoption across a broad set of health plans, providers, and purchasers. The CEP Work Group is collecting feedback in three ways on the draft White Paper and its specific recommendations: through an online discussion forum on Handshake, comments submitted directly on the LAN website, and via email at PaymentNetwork@MITRE.org In addition to collecting input to the draft elective joint replacement white paper, the CEP Work Group is simultaneously working on framing two additional episodes: cardiac care and maternity care. The Work Group is developing some preliminary recommendations, which will be shared in April. |

Please join us this afternoon as the LAN hosts a webinar presentation to discuss the CEP Work Group’s efforts to develop episode payment recommendations, including a new draft white paper entitled Accelerating and Aligning Clinical Episode Payment Models: Elective Joint Replacement. CEP Work Group Chair Lew Sandy will highlight the ways in which the APM model of episode payment can accelerate health care transformation, describe the Work Group’s charge, and discuss the organizational framework used to develop episode payment recommendations. With the release of the draft white paper on Friday, February 26, Lew Sandy will also describe the high level recommendations from the paper and solicit your questions and feedback. Work Group member Steven Spaulding will discuss the Arkansas Healthcare Payment Improvement Initiative, which uses episode payment to drive innovation in care delivery for a number of different procedures and conditions, including joint replacement. | ||||||

Priority Setting for 2016 and Beyond | ||||||
The Guiding Committee (GC) held a teleconference on Thursday, February 11 to recap the priorities for the LAN’s second year of operation that were discussed at the previous meeting, gain feedback on preliminary plans for standing up a new group focused on payment reform evaluations, and review the CEP Work Group’s progress.
In setting priorities for 2016, the GC emphasized that the focus should be on “action.” The LAN should build on the white paper recommendations by moving towards pilots in partnership with other organizations. The GC also emphasized that time is limited and that there is a need to prioritize carefully based on what’s achievable. The next step is to further shape the higher priority items. During the meeting, GC members also provided feedback on preliminary plans for forming a new work group to identify and develop steps for reducing cost and increasing the impact of evaluations of payment reform. The GC emphasized the importance of this work, noting that better evidence through more efficient and effective evaluation can lead to better payment reform design, more rapid diffusion of payment reforms, and better results for patients. This work would be supported by the Arnold Foundation and the Duke-Margolis Center for Health Policy. |

Payer Collaborative Discusses Pilot to Test Draft Metrics | ||||||
The LAN Payer Collaborative, composed of diverse public and private health plans, held its second virtual meeting on February 4. The Collaborative’s mission is to inform the LAN’s approach for measuring progress of APM adoption against the LAN’s goals of 30% adoption by 2016 and 50% adoption by 2018.The collaborative plans to conduct a five-week pilot project to test the feasibility and timing of the LAN measurement approach, developed by the APM FPT Work Group, as well as provide feedback on the metrics used to ensure data received is consistently interpreted and applied. Nine health plans committed to participate in the pilot project. The pilot project will inform a nationwide data collection effort to measure adoption of APMs in commercial, Medicaid, and Medicare Advantage market segments. Participants were invited to a training session on February 18, the pilot start date. The April 2016 LAN Summit will provide a potential opportunity for pilot project participants to share their experiences with other stakeholders. | ||||||

Less than Two Weeks Left to Provide Comments on Draft White Papers | ||||||
As a reminder, the public comment period for the first two draft white papers from the Population-Based Payment (PBP) Work Group is closing on Monday, March 7. If you’re interested in providing input or feedback on the patient attribution or financial benchmarking white papers or any of the diagrams, flowcharts, and tables used to illustrate key concepts, please weigh in. You can do so via Handshake, email at PaymentNetwork@MITRE.org, or the online patient attribution or financial benchmarking comment forms. We’re eager to hear your thoughts on these important topics and utilize the collective input from multiple stakeholders to help shape the final recommendations in these areas. With increased public-private engagement of these issues, we hope to accelerate the adoption of PBP models. Both white papers are targeted for release in final form in early April. | ||||||
